How we use it
To perform an initial service review, respond to you, protect the service and comply with legal obligations. We do not sell personal data or use submitted health information for advertising.
Storage and access
All assessment data submitted by users is stored in a secure database, with database access permissions only granted to designated case reviewers. The public-facing website is purely for display and assessment submission purposes and does not support account registration or login functions.
Retention and rights
Retention periods must be set by the operating entity before clinical onboarding. You may request access, correction or deletion using the contact method supplied in your case response.
International transfers
Cross-border handling may occur when a case requires coordination outside your country. Any transfer of medical records requires a separate notice and consent. Do not upload records through this form.
Contact
The operating entity’s legal name, registered address and privacy contact must be published before paid services begin.
